En base a la información obtenida se concluye que este elemento (cromo) es muy importante para facilitar el ingreso de la glucosa desde el exterior al interior de las células con lo que se reducen los niveles de glucosa en sangre, principal problema presente en personas diabéticas. Así mismo se recomienda una ingesta diaria de 100 a 200 mg de cromo día en aquellas pacientes con diabetes tipo II, para normalizar los niveles de glucosa en su organismo]]></p></abstract>

<body><![CDATA[<p align="justify"><font size="2"> Con la finalidad de dar a conocer la importancia del cromo en el organismo de personas que padecen diabetes no insulino dependiente (diabetes mellitus tipo II), se realiz&oacute; una extensa revisi&oacute;n bibliogr&aacute;fica sobre el tema. As&iacute; mismo se recomienda una ingesta diaria de 100 a 200 mg de cromo d&iacute;a en aquellas pacientes con diabetes tipo II, para normalizar los niveles de glucosa en su organismo. </font></p> </font>     <p> <font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>Palabras clave:</strong> Diabetes mellitus/ cromo trivalente/ factor tolerancia glucosa </font></p> <hr noshade="noshade" />     <p>&nbsp;</p>     <p>&nbsp;</p>     <p><font size="3" face="Verdana, Arial, Helvetica, sans-serif"><b>Introducci&oacute;n</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La diabetes mellitus fue reconocida antes de la era cristiana, tal es as&iacute; que en el papiro de Ebers descubierto en Egipto (siglo XV AC), se describieron s&iacute;ntomas que parecen corresponder a la diabetes. En el siglo II (despu&eacute;s de Cristo) Areteo de Capadocia dio a esta afecci&oacute;n el nombre de diabetes, que en griego significa sif&oacute;n refiri&eacute;ndose al signo mas llamativo de esta dolencia, que es la eliminaci&oacute;n exagerada de agua por el ri&ntilde;&oacute;n. </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> En 1679 Tomas Willis hizo una descripci&oacute;n magistral de la diabetes, quedando desde entonces reconocida por su sintomatolog&iacute;a como una entidad cl&iacute;nica claramente definida, as&iacute; mismo debido al sabor dulce de la orina le dio a esta afecci&oacute;n el nombre de diabetes mellitus (sabor a miel). En 1755 Dopson identific&oacute; la presencia de elevados niveles de glucosa en sangre en personas diab&eacute;ticas. En 1788 el Ingles Rollo logr&oacute; mejor&iacute;as notables en pacientes con esta dolencia al proporcionarles un r&eacute;gimen diet&eacute;tico rico en prote&iacute;nas, grasas y muy bajo en hidratos de carbono. En 1921 los Canadienses Banting y Best consiguieron aislar la insulina y demostraron su efecto hipoglucemiante, logrando de esta manera el avance m&aacute;s notable en la lucha contra esta enfermedad. A mediados del siglo XIX el cl&iacute;nico Franc&eacute;s Bouchardat se&ntilde;al&oacute; a la obesidad y la vida sedentaria de las personas como causas predisponentes para contraer diabetes. </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>La diabetes en Bolivia y el Mundo</strong></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La OMS (1997) report&oacute; que a nivel mundial el n&uacute;mero de personas afectadas por la diabetes ascienden a 135 millones, la misma que seg&uacute;n proyecciones espera se duplique para el a&ntilde;o 2025. De acuerdo con las previsiones este aumento ser&aacute; del 40 % en los pa&iacute;ses desarrollados y de casi el 170 % en aquellos que est&aacute;n en v&iacute;as de desarrollo. </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Un estudio realizado en La Paz, Cochabamba y Santa Cruz por la OPS en 1998, report&oacute; que el 7.2 % de las personas que viven en el eje central padecen de diabetes. Debido al aumento que esta experimentando esta enfermedad en Bolivia, el presente estudio tiene por objeto dar a conocer la importancia del cromo en la regulaci&oacute;n de los niveles de glucosa en sangre, en personas que padecen diabetes mellitus tipo II (no insulino dependiente). </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>El cromo en la naturaleza</strong></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El cromo como elemento mineral fue descubierto por Vaquelin en 1797, y le dio &eacute;ste nombre porque al tratarlo con el carb&oacute;n a temperaturas elevadas, daba lugar a un producto intensamente coloreado (Barceloux, 1999). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El cromo en el globo terr&aacute;queo permanece bajo varios estados de oxidaci&oacute;n, pero el cero (0), el trivalente (+3) y el hexavalente (+6) son los mas importantes en el ambiente y los productos comerciales. El cromo cero no esta presente en la corteza terrestre y es biol&oacute;gicamente inerte; el cromo hexavalente (Cr<sup>6+</sup>) es la segunda forma mas estable y es un fuerte agente de oxidaci&oacute;n especialmente en acides media; este elemento al reaccionar con material gen&eacute;tico de las c&eacute;lulas se convierte en un compuesto carcinog&eacute;nico (Cohen, 1993; Pechova y Pavlata, 2007). Casi todo el cromo presente en la aturaleza esta en forma de cromo trivalente (Cr<sup>3+</sup>), y se lo encuentra en la mayor&iacute;a de los materiales biol&oacute;gicos asociado fuertemente con prote&iacute;nas, &aacute;cidos nucleicos y otros componentes del organismo (Vercruysse, 1984; Anderson, 1997). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Algunas combinaciones del cromo tales como el &aacute;cido cr&oacute;mico, cromato de calcio, tri&oacute;xido de cromo, cromato de plomo, cromato de estroncio y cromato de zinc han demostrado ser sustancias altamente cancer&iacute;genas tanto para el hombre como los animales (Gerhardsson y Nordberg, 1993; Garcia, 2000). </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>Retrospectiva del cromo </strong></font><font face="Verdana, Arial, Helvetica, sans-serif"></font><font face="Verdana, Arial, Helvetica, sans-serif"> </font></p> <font face="Verdana, Arial, Helvetica, sans-serif"></font>    <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Mertz y Schwartz (1955; citados por Vincent, 2001) observaron que ratones de laboratorio que fueron alimentados con aplicaci&oacute;n intravenosa de glucosa, desarrollaron intolerancia a este carbohidrato y adem&aacute;s presentaban lesiones necr&oacute;ticas en h&iacute;gado. En 1957, Schwartz y Mertz reportaron que un compuesto al que luego denominaron como factor de tolerancia a la glucosa (FTG), y que estaba presente en la levadura de cerveza, era capaz de suprimir este problema. En 1959, Schwartz y Mertz identificaron al cromo como un componente activo del FTG; en este mismo a&ntilde;o los investigadores antes mencionados aislaron el FTG de un hidrolizado de levadura y el ri&ntilde;&oacute;n del cerdo, luego al introducir este componente v&iacute;a sonda al est&oacute;mago de ratones con intolerancia a la glucosa, observaron que se reestablec&iacute;a el metabolismo de esta aldohexosa. </font></p>     <p align="justify">&nbsp;</p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>Absorci&oacute;n del cromo como mineral inorg&aacute;nico y quelado </strong> </font> </p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El cromo inorg&aacute;nico (o cromo trivalente) es absorbido por el organismo del hombre y los animales en niveles muy bajos (0.4 a 3 % m&aacute;s o menos) y al parecer lo hace por difusi&oacute;n pasiva (Bunker et al., 1984; Anderson y Kozlowski, 1985; Anderson, 1987; Anderson, 1988; Stoecker, 1999). El NRC (1988) mencion&oacute; que la absorci&oacute;n promedio del cromo procedente de los ingredientes en una dieta normal es aproximadamente del 0.5 %. El cromo en forma de &oacute;xido (Cr2O3) no puede atravesar f&aacute;cilmente la membrana celular, por esta situaci&oacute;n es muy utilizado como marcador externo en pruebas in vivo para estimar la digestibilidad de algunos componentes nutricionales (Furnival et al., 1990a,b; Mertz, 1992; Ringo, 1993; De Silva et al. 1997). Varios estudios demostraron que dietas con elevados niveles de yerro y zinc, interfieren la absorci&oacute;n del cromo en intestino (Borel y Anderson, 1984; Mowat et al. 1995; Mowat, 1997). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El cromo en forma org&aacute;nica de quelatos es mas asimilado por el organismo (Gadd, 1995; Hemken, 1997); Chang et al. (1994) observaron que el coeficiente de absorci&oacute;n para el cromo quelado estuvo oscilando entre el 10 y 25 %. Un quelato puede definirse como un complejo formado entre un ligante y un i&oacute;n met&aacute;lico, donde el ligante (o agente quelante) debe contener grupos funcionales capaces de donar un par de electrones para poder combinarse a trav&eacute;s de una uni&oacute;n covalente con un metal (Langwinsky y Patino, 2001). Entre las formas de cromo org&aacute;nico (o quelado) mas utilizadas est&aacute;n el picolinato de cromo (Pic-Cr), el cromo niacina (Cr-Niac.), el cromo-levadura (Cr-Lev.) y el cromo metionina (Cr-Met.) (Merck &amp; Co., 1996). </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>El rol del cromo en el metabolismo de carbohidratos, l&iacute;pidos y prote&iacute;nas </strong></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Existen numerosas evidencias de que el cromo tanto en el organismo del hombre como en los animales posee una marcada influencia sobre el metabolismo de carbohidratos, l&iacute;pidos, prote&iacute;nas y &aacute;cidos nucleicos (Mertz, 1993; Anderson, 1994; Southern y Payne, 2003). Al formar parte de una mol&eacute;cula organomet&aacute;lica conocida como factor de tolerancia a la glucosa (<a href="#f1">Figura 1</a>), el cromo tiene una activa participaci&oacute;n en el metabolismo de los carbohidratos; este componente (FTG) act&uacute;a como un cofactor que potencializa la acci&oacute;n de la insulina elaborada por el p&aacute;ncreas (Steele et al., 1977; Anderson et al., 1983; Chirase et al.,1991; Evans y Pouchnik, 1993; Kegley et al., 2000)</font></p>     <p align="center"><a name="f1"></a><img src="/img/revistas/rtc/v5n5/figura_a02_1.gif" width="355" height="221"></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El mecanismo de acci&oacute;n del FTG a&uacute;n no esta muy claro, pero es probable que &eacute;ste act&uacute;e facilitando la uni&oacute;n de la insulina a los receptores celulares, y cuando esto sucede se facilita la penetraci&oacute;n de la glucosa al interior de las c&eacute;lulas (tejido hep&aacute;tico, muscular, adiposo) para que pueda iniciar con su metabolismo (anab&oacute;lico o catab&oacute;lico); por tanto este componente organomet&aacute;lico (FTG) contribuye a la obtenci&oacute;n de energ&iacute;a a partir de este carbohidrato, como tambi&eacute;n coadyuva en la regulaci&oacute;n de los niveles de glucosa en sangre, aspecto muy importante en personas con diabetes tipo II (Mooradian y Morley, 1987). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> La insulina que es secretada por las c&eacute;lulas beta de los islotes de langerhans (p&aacute;ncreas), tambi&eacute;n posee un efecto anab&oacute;lico muy importante (h&iacute;gado, m&uacute;sculo y tejido adiposo), y adem&aacute;s inhibe la gluoneog&eacute;nesis. En el primer caso (anabolismo), la glucosa una vez en el interior de la c&eacute;lula debe seguir diferentes rutas metab&oacute;licas para convertirse en gluc&oacute;geno, amino&aacute;cidos (no esenciales) o &aacute;cidos grasos; en el segundo (gluconeog&eacute;nesis) impide la obtenci&oacute;n de glucosa a partir de prote&iacute;nas y el glicerol (Steiner et al., 1967; Burton, 1995; Mertz, 1993; Guyton, 1997). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Como consecuencia de a&ntilde;adir cromo a dietas para hombres y animales tambi&eacute;n se observ&oacute; una disminuci&oacute;n en la concentraci&oacute;n de triglic&eacute;ridos y colesterol en sangre, pero el mecanismo mediante el cual este elemento (Cr) contribuye a reducir los componentes lip&iacute;dicos antes mencionados, a&uacute;n es desconocido (Lifschitz et al., 1980; Riales y Albrink, 1981; Mossop, 1983; Lefavi et al., 1993). </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Factores estresantes como el calor, ejercicios extenuantes (transporte), elevada ingesti&oacute;n de carbohidratos (glucosa), gestaci&oacute;n, infecciones y dietas con baja concentraci&oacute;n prote&iacute;ca, aumentan la excreci&oacute;n de cromo por orina (Borel et al., 1984; Anderson, 1988; Mowat et al., 1995). Una deficiencia de este elemento (hombre y animales) promueve un incremento en la presentaci&oacute;n de diabetes (Tipo II), glicosuria, altos niveles sangu&iacute;neos de placas formadoras de colesterol y dolencia coronaria cardiaca (Anderson y Mertz, 1977; Schrauzer et al., 1986; Anderson et al., 1991; Mertz, 1992; Anderson, 1994; Bunting et al., 1994). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Arag&oacute;n et al. (2001) en animales suplementados con Cr-levadura que permanecieron en ambientes c&aacute;lidos (31 y 35 &ordm;C) y con humedad elevada en el aire (70 y 78 %), observaron una significativa reducci&oacute;n del cortisol s&eacute;rico; sugiriendo por tanto que la suplementaci&oacute;n con cromo puede tener un efecto antiestr&eacute;s muy importante, ya que en situaciones de esta naturaleza (estr&eacute;s) los glucocorticoides en sangre normalmente suelen estar elevados (Anderson et al., 1997). </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><strong>Toxicocin&eacute;tica y metabolismo del cromo</strong></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif">El cromo puede absorberse por v&iacute;a digestiva, respiratoria y   cut&aacute;nea. Este elemento posee una gran afinidad por las   prote&iacute;nas org&aacute;nicas, a las que se fija formando compuestos   muy estables. El cromo trivalente y el hexavalente son agentes   desnaturalizantes de las prote&iacute;nas y precipitantes de los   &aacute;cidos nucleicos, se elimina muy   lentamente por la orina aunque en   pacientes con diabetes esta tiende a   incrementarse.</font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> En los tejidos biol&oacute;gicos, el cati&oacute;n   se encuentra fundamentalmente   como Cr3<sup>+</sup>, siendo en los pulmones   donde la concentraci&oacute;n de este cati&oacute;n   es m&aacute;s elevada. Producto de la   contaminaci&oacute;n los niveles de cromo   en los tejidos, ha demostrado ser   mayor en aquellas personas que   viven en grandes ciudades (Goyer,   1996).</font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2"><strong><font face="Verdana, Arial, Helvetica, sans-serif">Requerimientos de cromo</font></strong></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Con dietas normales el consumo promedio d&iacute;a de una persona es de 50 a 80 mg de cromo, cantidad insuficiente para el funcionamiento normal del organismo; la Academia Nacional de Ciencias de los Estados Unidos recomienda una ingesta diaria de 100-200 mg de cromo para el ser humano (Rosson et al. 1979; Bukhari et al., 1987; Preuss y Anderson, 1998). </font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> El suplemento de cromo es esencial para los seres humanos especialmente para los diab&eacute;ticos, ya que este tipo de pacientes presentan un metabolismo de cromo alterado y requieren mayores cantidades de &eacute;ste suplemento nutritivo debido a su mala asimilaci&oacute;n; por lo que los diab&eacute;ticos absorben m&aacute;s cromo que los no diab&eacute;ticos, pero sufren mayores p&eacute;rdidas de este elemento por la orina. La deficiencia de cromo es una consecuencia de la diabetes m&aacute;s que una causa y por eso se hace indispensable su suplemento en los pacientes diab&eacute;ticos, ya que pueden mejorar la tolerancia a la glucosa y los niveles de insulina (Anderson et al., 1997; Anderson, 1997). </font></p>     ]]></body>
<body><![CDATA[<p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> En las &uacute;ltimas tres d&eacute;cadas se ha demostrado en varios estudios, que el proporcionar las cantidades adecuadas de cromo en la dieta diaria de pacientes con deterioro de la tolerancia a la glucosa o con diabetes, da como resultado un mejoramiento del nivel de glucosa en sangre, de la insulina y de algunas variables lip&iacute;dicas (Mertz, 1993; Anderson et al. 1997). El mejoramiento nutricional del cromo junto con la insulina han disminuido los factores de riesgo de enfermedades cardiovasculares. Tambi&eacute;n se han normalizado los niveles de colesterol total, y colesterol de lipoprote&iacute;nas de alta y baja densidad, lo mismo que los triglic&eacute;ridos, tanto en animales de laboratorio, como en humanos, despu&eacute;s de suplementos de cromo (Ahmad y Khalid, 1987; Mertz, 1993; Thomas y  Gropper, 1996; Ravina et al.,1999). El Cromo (Cr), despu&eacute;s del Calcio, constituye el suplemento mineral m&aacute;s vendido en los Estados Unidos (P&eacute;rez, 2004). </font></p>     <p align="justify">&nbsp;</p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"><b>Influencia del cromo sobre la concentraci&oacute;n de glucosa,    colesterol y triglic&eacute;ridos en sangre del cerdo</b></font></p>     <p align="justify"><font size="2" face="Verdana, Arial, Helvetica, sans-serif"> Page et al. (1991), Toskano et al. (1999), Lemme et al.   (1999) y Garc&iacute;a et al.
