Obesity is a chronic, complex and multifactorial disease. In spite of taking precautions against it, in developed countries even in ours, it is an important cause of pulmonary and cardiac diseases. Its origin involves genetic and environmental factors, which determine a metabolic disorder as an excessive corporal fat accumulation that it is not an expected value according to sex, stature and age. Now, we present a case of a 52 years old masculine patient who relates a clinical condition that it presents at the beginning, a fit of hacking cough which becomes productive at fourth day, whose characteristics are white sputum in a moderate amount. It is enclosed with small efforts dyspnea, not quantified thermal rises and slight oppressive frontal headache, asthenia and adynamia; those are why his admit is decided. To complement, the breathing to low pulmonary volume gives rise to redistribution of the ventilation to superior pulmonary zones whereas the inferior zones of the lung have hypervascularization that it is causing an alteration of the ventilation-perfusion relation and hypoxemia.
